CPT Code 99453

Remote Monitoring of Physiologic Parameters

Includes: Initial setup and patient education of devices used for RPM to monitor vitals such as body weight, blood pressure, oxygen saturation, temperature etc. This code cannot be used if monitoring frequency is less than 16 days out of 30 days in a month. Billing Frequency: Billed only once. Medicare Reimbursement Rates: $21

CPT Code 99454

Remote Monitoring of Physiologic Parameters

Includes: Device supply with daily recordings and programmed alerts. This also includes collection, storage, and transmission of recorded data to healthcare providers. Billing Frequency: Once a month Medicare Reimbursement Rates: $69

CPT Code 99457

Remote Monitoring Treatment Management Services

Includes: 20 minutes of remote monitoring, treatment, and management services by a qualified healthcare professional. This involves two-sided and real-time interactive communication between the patient and healthcare provider.

Remote Monitoring of Physiologic Parameters

Includes: Additional 20 minute sessions of remote monitoring, treatment, and management services by a qualified healthcare professional. Billing Frequency: Once a month Medicare Reimbursement Rates: $43

CPT Code 99091

Collection and Interpretation of Physiologic Data

Includes: Non-interactive collection and analysis of physiological parameters such as blood glucose, blood pressure, ECG, etc. This data is electronically stored and transmitted to the healthcare provider. Billing Frequency: Once a month Medicare Reimbursement Rates: $59.19

CPT Code 99490

Chronic Care Management Services

Includes: A 20 minutes remote consultation used for monitoring a management plan. Billing Frequency: Once a month Medicare Reimbursement Rates: $42

CPT codes 99487, 99489, and 99490 for chronic care management services are also now covered by private providers offering Medicare Advantage, such as United Healthcare and Aetna.

In addition, the American Medical Association (AMA) has recently announced six new codes supporting RPM for 2020. These are 99421, 99422, 99423, 98970, 98971 and 98972.
